This policy covers the website only. Information you share with the practice as a patient — in person, by phone, or through the patient portal — is protected health information and is handled under our Notice of Privacy Practices (HIPAA), not this policy.

Information you send through the contact form

If you use the contact form, we receive the information you enter: your name, email address, phone number (if you provide it), preferred location, and your message. The form is delivered to the practice by email through Web3Forms, a third-party form service.

This form is not a secure channel for medical information. Please do not include medical history, symptoms, medications or other health information in your message. If you need to discuss your health or your care, please request an appointment or contact the practice directly.

We use what you send only to respond to your message. We do not add you to a mailing list.

Analytics

This website uses Google Analytics 4 to understand, in aggregate, how visitors use the site — for example, which pages are viewed, how visitors arrive at the site, the type of device and browser used, and approximate location as determined by Google.
